Registered Nurse

Liberty Care & Rehabilitation Center is seeking a compassionate Registered Nurse to provide high-quality bedside care to residents in our skilled nursing and rehab community. In this role, you will assess patients, administer medications and treatments, manage care plans, and document in EMR while ensuring safety, comfort, and dignity. You'll collaborate closely with an interdisciplinary team, supervise CNAs, and educate residents and families.

Responsibilities

• Deliver direct nursing care to residents, including assessments, treatments, and medication administration
• Develop, implement, and update individualized care plans in collaboration with the interdisciplinary team
• Monitor resident conditions, document in EMR, and promptly report changes to providers and families
• Support safe transitions of care, including admissions, discharges, and transfers
• Educate residents and families on care plans, treatments, and health management
• Maintain infection control, safety, and quality standards in alignment with facility policies
• Supervise and support CNAs and other care staff to ensure high-quality resident care
• Participate in care conferences, rounds, and quality improvement initiatives
