Role: Licensed Practical Nurse (LPN) providing in-home and school-based care for Pediatric and Adult patients.
Responsibilities- Assess, plan, implement, and evaluate patient care using the nursing process.
- Deliver skilled nursing services per prescribed orders.
- Monitor signs and symptoms indicating physiological and psychosocial changes.
- Collect, analyze, and interpret healthcare data, documenting diagnoses and patient status.
- Licensed as an LPN/LVN in the practicing state.
- Current TB or Chest X-Ray and BLS certification.
- Preferred: one year of professional nursing experience.
Strong assessment abilities, adherence to medical orders, documentation, and the capacity to identify patient condition changes. Competence in providing compassionate, skilled nursing care in diverse settings.
