Physical Therapist

We are hiring skilled and compassionate physical therapists to join our home health interdisciplinary team! You'll work in a home setting with a diverse patient population throughout these surrounding communities. Working outside of a medical facility, you'll have an opportunity to get to know patients as you guide their care in the comfort of their homes.

Schedule:

Monday - Friday, 8:00 am - 4:30 pm (no on-call), Weekend rotations

• Perform duties in accordance with system-wide competencies and behaviors. Attend compliance training and adhere to organizational policies and procedures.
• Focus on customer service to ensure patient satisfaction.
• Identify and participate in process improvement initiatives to enhance service quality.
• Perform duties aligning with the service management philosophy of UPMC Home Healthcare.
• Engage in ongoing professional development through continuing education.
• Participate in on-call coverage rotations.
• Supervise the Physical Therapy Assistant according to state guidelines.
• Consult with families, interdisciplinary team members, physicians, and other professionals to ensure optimal patient care.
• Transmit information promptly and accurately, including visit assessments, OASIS, 485, and verbal orders.
• Establish the plan of care collaboratively with the physician, patient, and UM professional, following the admission process.
• Evaluate, plan, and treat patients using diagnostic and functional ability testing, and recommend necessary DME.
