• Conduct comprehensive patient assessments, evaluating physical conditions, functional limitations, and medical histories to develop individualized treatment strategies
• Design and implement customized rehabilitation treatment plans tailored to each patient's specific needs, goals, and home environment
• Perform therapeutic exercises and mobility interventions to improve strength, flexibility, balance, and functional independence
• Provide patient and family education on injury prevention, post-operative recovery, chronic condition management, and home safety modifications
• Monitor and document patient progress regularly, adjusting treatment plans as needed to optimize outcomes
• Collaborate effectively with physicians, nurses, occupational therapists, and other healthcare professionals to ensure coordinated, comprehensive patient care
• Maintain detailed, accurate, and HIPAA-compliant clinical documentation and patient records
• Manage caseload efficiently while traveling to patients' homes throughout the service area

• Master's degree or Doctorate in Physical Therapy from an accredited program
• Minimum of 1 year of professional experience as a Physical Therapist in home health care settings
• Current, valid Physical Therapy license in the state
• Current CPR/BLS (Basic Life Support) certification
• Valid driver's license with reliable transportation and ability to travel to multiple patient locations daily
• Strong clinical assessment and treatment planning skills
• Excellent communication and interpersonal abilities to work with diverse patient populations
• Proficiency with electronic medical records (EMR) and documentation systems
