Responsibilities- Be at the center of patient flow, partnering with referring and accepting physicians to coordinate smooth, efficient transfers between facilities.
- Work closely with attending physicians and inpatient charge nurses to ensure patients are placed quickly and appropriately, aligning with care needs and placement guidelines.
- Manage inpatient flow of patients from preadmission and admission through discharge, optimizing utilization of inpatient beds and identifying demand/capacity mismatches.
- Assess, plan, and facilitate inpatient transfers in conjunction with clinicians and interdisciplinary team members.
- Provide clinical screening on all admission requests using Interqual Criteria for medical necessity, ensuring appropriateness and pre-certification.
- Collaborate with utilization management for follow-up on urgent and emergent transfers and admissions.
- Document communication notes in the electronic medical record, including reason for transfer and transfer plan details.
- Coordinate admission details with departments and facilitate bed assignments based on acuity, aggregation, and bed availability.
- Communicate with attending physicians for issue resolution or intervention.
- Monitor admission processes and mitigate any delays.
- Identify quality management issues and report to appropriate manager.
- Serve as a resource to physicians and hospital personnel regarding system-wide inpatient bed availability.
- Direct and alter assignments in response to changes in capacity and acuity of departments.
- Participate in coverage and on‑call systems per department policy.

Qualifications- Graduate from a Specialty Training Program in Nursing (required).
- Minimum of 3 years of nursing experience.
- Licensed Registered Nurse in Pennsylvania ( of Pennsylvania).
