We’re seeking a dedicated nursing professional to contribute to patient education and comprehensive care within a multidisciplinary team.
Responsibilities- Provide patient care and education for patients in the specific age range on their assigned department.
- Serve as a resource for patients and families regarding the perception of diagnosis and medical plan of care for disease management as designated by the physician.
- Maintain availability to nursing for occasional in-service and assist with orientation of staff involved in education.
- Perform pre-visit data gathering and record keeping on new and return patients, including recording current medications and dosages and obtaining previous laboratory results and measurements.
- Assist in the management of patients with chronic diseases, including achieving key quality indicators, education, and long-term coordination of care.
- Track performance improvement measures and assist providers with completion.
- Obtain and document patient medical history, chief complaint, vital signs, and provide basic medical data for the provider; prepare the patient for examinations.
- Assist the provider in the performance of procedures as directed to facilitate accurate diagnosis.
- Collect and prepare laboratory specimens and complete associated paperwork to expedite laboratory studies with maximum accuracy and efficiency.
- Assist with orientation of new personnel to ensure appropriate training to support patients and providers.
- Execute and document ongoing patient communication and provide instruction to ensure patient understanding of medical condition and compliance with treatment instructions.
- Insert, monitor, regulate, and discontinue IVs as required by the provider.
- Perform and assist with triaging telephone calls concerning health issues and provide telephone management advice as a liaison to physician recommendations.
- Communicate lab, radiology, and other test results to patients via telephone and electronic medical record notifications.
This role is full-time, 40 hours weekly;
Monday through Friday; hours rotating between 8:00a-4:30p and 8:30a-5:00p. Work is typically performed in a clinical environment. Education:
Graduate from Specialty Training Program-Nursing (Required)
Experience & Licenses:
Licensed Registered Nurse (Pennsylvania) - of Pennsylvania;
Basic Life Support Certification
