Registered Nurse Navigator Triage – Population Health

Role Overview:

The RN Nurse Navigator Triage provides nursing triage and health advice via phone and in person, assessing patient acuity and determining medical needs. They ensure continuity of care by delivering results, education, and protocol-driven clinical advice, while managing timely communication and documentation.

Perform patient assessments, prioritize urgent calls, initiate emergency treatments, and coordinate care across departments and external organizations. They educate patients and families, schedule appointments, support new staff training, and manage patient concerns to ensure satisfaction. Additionally, they facilitate communication between patients, providers, and other healthcare entities, ensuring proper documentation and continuous improvement.

A graduate from a specialty nursing program is required; BSN is preferred. Five years of nursing experience, especially in ICU, ED, or critical care, is strongly preferred. Proficiency with computer systems and Microsoft Office is essential. Certification in specialty areas is advantageous.

Typically performed in a clinical setting, adhering to organizational policies. The role involves flexible shifts, including days and nights, with some travel for training or meetings.
