Registered Nurse Navigator – ENT

Provides patient care coordination, education, and ongoing support within a multidisciplinary care program. Acts as a liaison between patients and healthcare providers, facilitating entry into care programs and ensuring continuity of care.

• Coordinates pre- and post-hospital care and multidisciplinary clinics.
• Guides patients through the care process as a patient navigator.
• Provides education to promote understanding of conditions and ongoing care needs.
• Supports diagnosis and treatment by collaborating with healthcare teams.
• Communicates with patients via mail, phone, and in person, and coordinates activities across clinics and support services.
• Maintains systems for timely follow-up and surveillance of patients.
• Assists with nursing care, procedures, and educates team members.

Graduate from a specialty training program (required), Bachelor's Degree in Nursing (preferred). Strong communication, coordination, and patient education skills are essential.
