Registered Nurse Navigator Triage - Population Health

The RN Nurse Navigator Triage provides nursing triage services and health advice via phone and in person, assessing patient acuity and determining medical needs. Responsibilities include documenting patient information, providing protocol-driven clinical advice, coordinating emergency and routine care, and educating patients and families. The role involves scheduling appointments, facilitating communication across departments, supporting new staff, and ensuring timely documentation. The position emphasizes patient satisfaction, care coordination with external organizations, and continuous quality improvement.

A graduate from a specialty nursing program is required, with a preference for a Bachelor's Degree in Nursing. Five years of nursing experience, especially in ICU, ED, or Critical Care, is strongly preferred. Proficiency in computer skills and Microsoft Office Suite is essential.

• Initiate emergency treatment when needed and prioritize urgent calls.
