Responsibilities- Conduct medical assessments
- Assess physical health needs
- Schedule and transport individuals to medical appointments
- Provide direct care to individuals
- Maintain accurate documents
- Must be a Licensed Practical Nurse with a valid LPN license in Virginia
- Must have a valid driver’s license and safe driving record
- Fingerprints, State and FBI criminal records, drug test, and CANIS report will be required upon request; the agency maintains a drug‑free workplace.
Hourly rate range: $22.56 – $39.48
Shift differential: $2.00 for 2nd shift, $3.00 for 3rd shift
