  • Coordinate communication among clients, families, referral sources, providers, and clinical staff
  • Assist with referrals to appropriate mental health, substance use, medical, housing, and community resources
  • Track pending admissions and follow up on missing documentation or outstanding requirements
  • Support transitions between outpatient, IOP, PHP, peer support, and other services as appropriate
  • Maintain accurate and timely documentation in the electronic health record
  • Participate in treatment-team communication and care coordination
  • Help identify barriers that may prevent clients from engaging in treatment
  • Maintain confidentiality and professional boundaries
  • Follow applicable Virginia DBHDS, DMAS, organizational, and CARF requirements
What We Are Looking For
  • Bachelor's degree in psychology, social work, counseling, human services, sociology, or a related field preferred
  • 1-3 years of behavioral health, substance use, intake, case management, or care coordination experience preferred
  • Strong communication and customer-service skills
  • Excellent organization and attention to detail
  • Ability to manage multiple referrals and priorities at the same time
  • Strong documentation and follow-up skills
  • Comfortable communicating with clients, families, providers, referral sources, and insurance representatives
  • Basic understanding of behavioral health and substance use treatment services
  • Ability to work effectively as part of a multidisciplinary team
We Would Especially Like to Meet You If You Have

  • Virginia Medicaid or DMAS experience
  • Insurance eligibility or authorization experience
  • Experience with mental health and substance use services
  • Experience coordinating IOP or PHP admissions
