Licensed Practical Nurse in Home Health
This role involves delivering high quality, technical patient care in clients' homes while coordinating with the health care team.
Primary Responsibilities- Provide clinical services within the scope of nursing practice, according to state laws and patient care plans, and follow infection control procedures.
- Document patient visits fully in the electronic medical record and the agency's electronic visit verification program.
- Participate in care coordination, report patient needs and changes, attend case conferences, and communicate effectively with families, physicians, other clinicians, and agency staff.
- Work safely and respectfully in any home situation, regardless of client background.
- Current, unrestricted LPN licensure in the state of practice.
- Valid driver’s license, vehicle insurance, and reliable transportation (or access to public transit).
- CPR certification or ability to obtain within 90 days of hire.
- Experience in home care.
- Ability to manage multiple tasks, work independently, and communicate effectively.
- Flexible hours to meet client needs.
Annual total cash target pay: $43,900 – $97,100 (full-time, 40 hrs/week). Compensation includes per‑visit point pay ($25.33 – $56.02 per point) and hourly rate ($21.11 – $46.68). Benefits include a comprehensive benefits package, recognition programs, equity stock purchase, and 401(k) contribution.
