Commonwealth Hospice, part of the LHC Group family and United Health Group, is seeking a Registered Nurse Case Manager for home visits. You will deliver the Plan of Care for hospice patients, collaborate with professionals, and promote physical, psychosocial, emotional, and spiritual well-being.
You will work full-time with on-call rotation evenings/weekends. VA RN licensure and reliable transportation are required; the agency covers a 50-mile radius for patient visits.
