Commonwealth Hospice, part of LHC Group and United Health Group family, is seeking a Registered Nurse Case Manager to deliver and coordinate the Plan of Care for hospice patients in home settings.
You will perform initial nursing evaluations within 48 hours of referral, develop the care plan with the care team, and ensure quality, compassionate care within a 50 mile radius of your assigned area. VA RN license and reliable transportation required.
