POSITION SUMMARY
The Crisis Licensed Practical Nurse (LPN) provides nursing support to individuals receiving behavioral health crisis stabilization services within Envision Health Inc.’s crisis program. The LPN works as part of an interdisciplinary crisis team to promote client safety, stabilization, medication continuity, and appropriate transition to the next level of care.
This position is scheduled for 12-hour shifts every other weekend. The LPN must remain available and engaged throughout the assigned shift and perform duties within the LPN scope of practice, Envision Health Inc. policies, applicable Virginia Board of Nursing requirements, and crisis-program regulations.
ESSENTIAL DUTIES AND RESPONSIBILITIES- Provide nursing services to individuals admitted to the crisis stabilization program within the LPN scope of practice.
- Obtain and document vital signs, health information, medication history, allergies, and other required nursing information.
- Observe clients for changes in physical condition, mental status, behavior, withdrawal symptoms, medication reactions, or other clinical concerns.
- Promptly report significant changes or concerns to the appropriate RN, physician, nurse practitioner, or other authorized clinical provider.
- Administer medications as ordered and within the LPN scope of practice.
- Maintain accurate medication administration records (MARs).
- Monitor clients for medication effectiveness, adverse reactions, and potential side effects and report concerns appropriately.
- Assist with medication reconciliation and verification according to agency procedures.
- Follow medication storage, security, inventory, disposal, and controlled-substance procedures.
- Provide basic nursing interventions and first aid when appropriate.
- Assist the crisis team in maintaining a safe and therapeutic environment.
- Participate in crisis response and emergency procedures when necessary.
- Coordinate with the RN and other members of the interdisciplinary team regarding client needs.
- Communicate relevant nursing information during shift changes and handoffs.
- Assist with admissions, transfers, and discharges as assigned.
- Complete required documentation accurately and within established time frames in the electronic health record.
- Maintain confidentiality and comply with HIPAA requirements.
- Follow infection-control, blood borne-pathogen, emergency-preparedness, medication-management, and workplace-safety procedures.
- Participate in required staff meetings, training, competency assessments, and continuing education.
- Immediately report serious incidents, medication errors, injuries, safety concerns, or other reportable events according to agency policy.
- Perform other nursing-related duties consistent with the LPN scope of practice and needs of the crisis program.
This position is specifically designated for every-other-weekend coverage.
The employee will generally work: 12-hour shifts every other weekend
Specific shift hours (for example, 7:00 AM–7:00 PM or 7:00 PM–7:00 AM) will be established based on program staffing needs.
Because this position provides scheduled crisis-program coverage, reliable and punctual attendance is an essential function of the position. The employee must follow Envision Health Inc.’s call-out and attendance procedures when unable to report for an assigned shift.
The employee may be offered additional shifts based on staffing needs and employee availability.
