Primary Care Physician (Full-Time)
Schedule:
Monday-Friday, 8:00 AM - 5:00 PM (No weekends or hospital rounds)
Setting: 100% Outpatient (Geriatrics-focused position)

We are seeking a compassionate and dedicated Primary Care Physician to deliver high-quality, value-based care to an established panel of predominantly geriatric patients. This role emphasizes preventive medicine, chronic disease management, and building meaningful, long-term patient relationships in a supportive outpatient environment. You will work within a collaborative, multidisciplinary care model designed to improve patient outcomes, enhance access to care, and reduce unnecessary hospitalizations.

- Provide comprehensive primary care services, including evaluation, diagnosis, and treatment of acute and chronic conditions.
- Deliver preventive care, wellness visits, and health screenings with a focus on early intervention.
- Develop and manage individualized care plans for a predominantly geriatric population.
- Collaborate closely with nurse practitioners, care coordinators, and support staff to ensure seamless, patient-centered care.
- Utilize electronic health records (EHR) to document clinical encounters and track patient outcomes.
- Engage in quality improvement initiatives and contribute to a culture focused on continuous care enhancement.
- Educate patients and families on disease prevention, treatment plans, and healthy lifestyle choices.
- MD or DO from an accredited medical school
- Internal Medicine or Family Medicine or Geriatric Medicine
- Active, AL license
- Current DEA registration
- At least 3 years of experience in Outpatient Primary Care
- Strong clinical judgment with a focus on preventive and longitudinal care
- Excellent interpersonal and communication skills
