If so, join our team at Legacy Hospice as a Triage Hospice Nurse and make a lasting impact on the lives of those in need!
Key Responsibilities:- Answer all incoming calls and communications and provide telehealth assessments, pain management, and symptoms.
- Assess the patient and family's need for a home visit and scheduling through HCHB and notify the patient and family of the expected visit time.
- Managing HCHB workflow including documentation of coordination notes and back office workflow.
- Serves as an after-hours point of contact to ensure referrals are documented and ensure timely admissions.
- Coordinate patient care and communication with interdisciplinary teams.
Qualifications:
- Practical Nurse, Vocational Nurse, or Registered Nurse licensed in the state of employment
- Certified Hospice and Palliative Nurse and/or ASN/BSN preferred
- Minimum 3 years of hospice nursing experience
- Remote access Wi-Fi and phone
- Demonstrated expertise in pain management, symptom control, crisis intervention, and emotional and pre-bereavement support
- Excellent critical thinking skills and clinical judgement
- Previous experience of operating and documenting in Homecare Homebased
- Excellent verbal and written communication skills
- Self-directed and able to work with minimal direct supervision
- Ability to work outside of posted regular business hours, including overnights and weekends
