Position Summary

The Intake Coordinator, RN is a dual-impact role responsible for driving the operational success of the opportunity funnel while providing clinical oversight. This individual streamlines the onboarding experience, navigating families through the complexities of the Connecticut Medicaid waiver process and orienting them to Careforth’s digital tools. Acting as a central liaison between families, referral agencies, and internal care teams, this role is vital in ensuring all nursing assessments, regulatory documentation, and payer authorizations are executed with precision.

What You Will Do
  • Manage the daily workflow of the consumer intake funnel, tracking opportunities from referral; this includes providing initial orientation to Careforth’s digital communication platform to ensure a seamless onboarding experience
  • Perform initial nursing assessments for prospective consumers, ensuring adherence to CT Medicaid waiver guidelines
  • Oversee the collection and verification of key documentation, including caregiver paperwork, background checks, and pre-authorization approvals
  • Monitor the status of prior authorizations from payer sources and case managers; proactively conduct outreach to resolve delays and maintain momentum in the pipeline
  • Ensure all intake activities and documentation remain in strict compliance with Medicaid waiver program regulations, Careforth policies, and federal, state, and local laws
  • Respond to requests or grievances from caregivers and consumers and communicate with team members regarding concerns or emerging issues
  • Partner closely with Enrollment, Sales, and Care Management Teams to align on goals and expedite the transition of consumers into active care
  • Foster and maintain professional relationships with referral agencies to optimize communication and drive referrals
  • Perform other duties and special projects as assigned
What You Will Bring
  • Must hold an active Registered Nurse (RN) License in the state of employment.
  • Minimum two (2) years of recent experience in direct care of elders or adults with disabilities.
  • Proven ability to manage internal workflows and systems in a fast‑paced environment.
  • Tech savvy with the ability to navigate case management systems and Microsoft Office.
  • Ability and flexibility to travel throughout Connecticut (usually by automobile); valid driver’s license and automobile insurance required.
  • Experience in case management, care planning, and assessment for cases including, but not limited to, eldercare, people with disabilities, and other complex medical needs in a healthcare setting.
  • Additional state‑specific qualifications may be required.

The pay range for this position is $79,587 - $97,464. The actual wage offered may be lower or higher depending on budget and candidate experience, knowledge, skills, qualifications, and geographic location.
