Registered Nurse - Home Health - SAT/SUN/MON

RVNAhealth is seeking a skilled Home Health Registered Nurse for a modified "Baylor" schedule (Sat & Sun plus one weekday). This RN provides direct patient care in the community setting, covering admissions, routine visits, and urgent needs. The role also supports weekday RN case managers by handling visit overflow.

This is a 30-hour position paid at full time, to include three (3) 10-hour shifts on Saturday, Sunday + one weekday (flexible).

This position will cover the central and northern territory of Fairfield County.

• Perform in-home nursing assessments and develop individualized care plans.
• Complete admissions, scheduled visits, and urgent patient needs on weekends.
• Support the weekday home health team by covering visit overflow.
• Deliver skilled nursing services, including wound care, medication administration, IV therapy, and chronic disease management.
• Coordinate the implementation of the plan of care for clients residing in SNF, ALF, hospital contract bed or personal residence.
• Monitor and document changes in patient condition; update care plans as needed.
• Communicate effectively with physicians, care teams, and family members.
• Ensure timely and accurate documentation in the electronic medical record.
• Participate in the rotating on-call schedule as needed.

• Graduate of an accredited nursing program; BSN preferred.
• Current RN license in the state of Connecticut.
• Minimum 2 years of nursing experience; home health experience strongly preferred.
• Proficient in using electronic medical records and mobile documentation tools.
• Strong clinical skills and ability to work independently in a community setting.
• Valid driver's license and reliable transportation for local travel.

Compensation: The pay range listed represents RVNAhealth's commitment to pay transparency and is a reasonable estimate of possible compensation for this role at the time of posting. The final salary for this role will be determined based on careful consideration of a variety of factors, such as the candidate's exact location, experience, and expertise related to the qualifications.
