Position: Registered Nurse (RN) - Home Health - $75K-100K per year

Job Overview

Graham Healthcare Group is seeking a Registered Nurse (RN) Home Health to support patients in Downers Grove, IL and surrounding areas. This full‑time position is ongoing, 40hours per week, 8‑hour shifts with a 4‑month orientation pay guarantee.

Key Responsibilities
  • Lead and collaborate to provide management and delivery of patient care plans as a home health registered nurse case manager.
  • Provide quality patient care and maintain timely charting to meet critical deadlines.
  • Self‑manage time and schedule, ensuring consistent availability for patient visits.
  • Use tablets, mobile phones, and EMR software to document and coordinate care.
Qualifications
  • Current RN license in the state of employment.
  • Graduate from an approved school of practical nursing.
  • At least one year of RN experience with clinical competence in patient care.
  • Valid driver’s license and willingness to spend ~20% of the day traveling to patient locations.
  • Proficiency with tablets, mobile phones, and EMR software.
  • Commitment to meet critical documentation deadlines consistently.
