RN Case Manager – Davenport and Kissimmee Home Visits
Compassionate Care Hospice (CCH), an Amedisys Company, is seeking an RN Case Manager for home visits. This position is based in Lakeland, FL and requires daily travel to the assigned area.
Schedule:
Monday‑Friday, 8:00 AM – 5:00 PM with participation in the on‑call rotation.

$82,000–$86,000 annually, plus bonus opportunities. Final compensation may vary based on experience and skills.

You will then continue your onboarding working directly with nurses in our program. On average our orientation for an RN is 7-9 weeks, however is individualized according to your needs and experience.
Responsibilities- Perform patient assessments and collaborate with the care team to develop and implement a plan of care.
- Makes referrals to other hospice care disciplines as needed or documents rationale for not doing so.
- Develop, prepare, and maintain individualized patient care progress records with accuracy, timeliness and in accordance with care center policies.
- Record pain/symptom management changes/outcomes as appropriate.
- Teach patients, families, and caregivers appropriate patient care techniques.
- Regularly communicate with the clinical manager and care team.
- Perform on‑call responsibilities and on‑call services to patients/families as assigned.
- Other duties as assigned.
- Current RN license in the state(s) where you will work
- Minimum 1 year of clinical RN experience in an acute healthcare setting (Required)
- Current CPR certification
- Hospice or home health experience
- Strong communication and interpersonal skills
- Ability to work independently and manage multiple priorities
- Comfortable with daily travel between facilities
- Proficient in electronic medical record (EMR) systems
