The Case Manager is responsible for the coordination of health care decisions by using a systematic approach to assure treatment plans that improve quality and outcomes, coordination of care across the continuum; promotion of cost-effective care within the allotted time frame; assuring payments of hospital-based services meeting patient-related utilization management criteria, and implementation of safe and appropriate discharge plans.
Daily Responsibilities- Oversee two case managers
- Implementation of safe and appropriate patient discharge plans
- Staff training and development
- Utilization Management and Review
- Financial Case Management (connecting the under-insured/uninsured to financial assistance)
Monday - Friday 8:00am – 5:00pm
Advantages of this Opportunity- Competitive salary, $70,000-$75,000 negotiable based on relevant experience

- 4-5 years Hospital Case Management experience with some type of critical care/medically complex case background within the last 3 years
