Full-Time Nurse – Outpatient Services

full-time Nurse to join our Outpatient team!

In this role, you will assess client care needs, provide clinical treatment, and coordinate medication services in collaboration with our on-site prescribers. This is a great opportunity for nurses who value meaningful work and a healthy work-life balance.

• Monday–Friday schedule
• 8-hour daytime shifts

• Provide nursing care that promotes, maintains, and restores physical and mental health
• Assess client needs and develop nursing diagnoses and treatment plans
• Monitor client responses and update treatment plans as needed
• Coordinate communication between patients, prescribers, and pharmacies
• Collaborate with healthcare providers and Integrated Health Home staff to ensure comprehensive care
