This Licensed Practical Nurse (LPN) position provides professional nursing care for residents of all ages in long‑term care under the supervision of a registered nurse, advanced practice provider, or physician.

Demonstrates knowledge of nursing principles for residents in the relevant clinical setting. Provides competency and practices within the full scope of nursing expertise, utilizing appropriate age‑and‑population specific standards as per care plan. Performs testing to evaluate resident status and provides required care. Identifies and responds to problems, recommends solutions, and assists in emergent situations. Ensures resident health, comfort, and safety. Contributes to assessment and administers medications or treatments as ordered.
Provides technical support to health‑care professionals. Documents resident interactions and outcomes related to care, medication, behavior, activity, and diet. Establishes and maintains effective working relationships with residents, health‑care providers, and the public. Works collaboratively with interdisciplinary team members. Communicates clearly and maintains confidentiality of sensitive information. Counsels residents, family members, and/or resident advocates on prevention and treatment plans under the direction of a registered nurse, advanced practice provider, or physician.
Develops self‑care programs based on resident education principles. Performs related duties as assigned.
Graduate from an accredited practical nursing program as an LPN; associate degree in practical nursing preferred. If the position requires IV therapy/management, must have graduated from a Board approved AASPN program including IV therapy or completed an approved IV Therapy course. Must have completed a pre‑licensure nursing program with supervised clinical experience across the lifespan or have a minimum of 400 hours of LPN experience at another facility.
Currently holds an unencumbered LPN license with State Nursing board and/or possesses multi‑state licensure privileges. Obtains and maintains required departmental competencies and certifications. Completes required annual CEU and competency training.
