RN Case Manager

• Conduct initial client assessments and continuously refine care plans
• Provide hands-on nursing care and patient education
• Maintain open lines of communication with patients, families, and the healthcare team
• Manage patient care coordination for your caseload
• Participate in interdisciplinary team meetings to ensure comprehensive care planning
• Make patients' final days, weeks, and months as comfortable as possible
• Work closely with the company's owners
• Contribute to internal growth and clinical advancement

• Licensed as a Registered Nurse / RN in the state you work in
• Proficiency with Electronic Medical Records (EMR) software
• Excellent planning, organizing, and communication skills
• Ability to solve problems autonomously while maintaining high standards
• Valid Driver's License
• Reliable vehicle with current insurance
