Job SummaryThe Welcome Baby:
Road to Resilience (WB:
R2R) Registered Nurse (RN) provides in‑home nursing care to postpartum Yolo County residents and their babies, including physical assessments, screenings, education, and connection to primary healthcare, behavioral healthcare, and community resources. The RN also offers telephone follow‑up as needed, performs WB:
R2R home visits, and serves as a primary care RN at CCOLE when assigned.
Not benefit eligible at 0.5 PTE.

MinimumPosition Requirements
- Education:
- Completion of an accredited RN Program
- Current California RN license and good standing with the Board of Registered Nurses
- CPR Certification required
- Bachelor’s of Science in Nursing or higher degree preferred
- Experience:
- At least one year of experience working with perinatal populations required
- At least one year of experience working with low‑income populations in a clinic setting strongly preferred
- Special Skills/Training:
- Current lactation certification (CLC/CLE or higher) and/or ability to achieve lactation certification within 6 months of hire
- Bilingual English/Spanish strongly preferred
- BLS Certificate Required
- Drivers License Required
- Apply nursing process, scope of practice, and standards of care for WB:
R2R clients in their homes. - Consult with staff medical providers and accept/transpose verbal or telephone orders.
- Provide patient care for Communi Care+OLE primary care patients in clinic settings under staff direction.
- Utilize the nursing process (assessment, diagnosis, planning, intervention, evaluation) in daily activities.
- Deliver patient education on infant care, immunizations, postpartum mood and anxiety, sexual activity and birth control, appropriate medical cueing, and babywearing.
- Participate in lactation training and maintain lactation certification; provide lactation assistance and coordinate with IBCLC and other resources.
- Schedule WB:
R2R clients for postpartum and well‑baby follow‑up appointments. - Process clinical patient information and WB:
R2R data collection in the electronic health record. - Participate in quality improvement projects assigned by the perinatal health coordinator or Associate Director.
- Assist with coordination of specialty care, referrals, and follow-up for complex medical problems.
- Coordinate care for additional services (dental, behavioral health, diabetes education, etc.).
- Perform independent face‑to‑face or telephone triage using written protocols for current/past WB:
R2R clients and CCOLE patients. - Attend WB:
R2R, clinic, and general Communi Care+OLE meetings as required. - Perform other duties as assigned.
