HHS) is seeking an experienced RN to conduct SOC admissions, perform follow-up evaluations, and visit patients in the Yolo to West Sacramento region. You will work with a clinical team to deliver high-quality home health care and manage medications, wound care, and IV infusions.
Must have CA RN license, BLS/CPR, and a valid CA driver's license. Experience with OASIS and high-acuity patients is preferred; IV therapy proficiency is a plus.
