Hospice Social Worker – Compassionate Care Role

Join our mission to provide holistic, end-of-life care through skilled social work support.

Role Purpose

The Hospice Social Worker provides emotional, practical, and spiritual support to individuals and families facing terminal illness, chronic illness, or other life-limiting conditions. They work closely with patients, families, and healthcare teams to address emotional, social, and practical needs, ensuring dignity, comfort, and informed decision-making throughout the end-of-life process.

Key Responsibilities
  • Conduct comprehensive assessments of patients and families to identify emotional, social, and practical needs
  • Develop and implement individualized care plans addressing psychosocial, spiritual, and end-of-life care goals.
  • Provide grief counseling, crisis intervention, and bereavement support for patients and families.
  • Facilitate communication between patients, families, and healthcare professionals to coordinate care.
  • Educate patients and families on end-of-life options, advance care planning, and available resources.
  • Assist with accessing community resources such as financial aid, legal services, housing, and transportation.
  • Advocate for patients’ rights and ensure compliance with federal and state hospice regulations.
  • Maintain accurate, up-to-date patient records and documentation.
  • Facilitate family meetings to discuss care goals, expectations, and end-of-life decisions.
Qualifications & Skills
  • Master’s degree in social work (MSW) from an accredited institution.
  • Licensed Clinical Social Worker (LCSW) or equivalent state licensure
  • Experience in hospice or palliative care settings (preferred).
  • Proficiency in crisis intervention and grief counseling techniques
  • Strong understanding of end-of-life issues, grief processes, and hospice regulations.
  • Exceptional communication, interpersonal, and organizational skills
  • Ability to work collaboratively within an interdisciplinary healthcare team
