Hours:The hours for this part-time position is 24 hours per week and the preferred schedule is 3 days per week.

The Social Worker is responsible for providing assessments, planning, interventions and follow-up for patients and family members at Shriners Children's across the continuum of care. Through this function, by joining the team, you ll have the opportunity to make an impact on our patient's experience. You ll help to provide continuity of care for positive outcomes in our family centered environment.

Required:

• Masters in Social Work
• Ohio Social Work license
• Case Management Certification (CMC, ACM or ANCC) required or the ability to attain certification within 12 months of hire or within 12 months of eligibility
• Three years of social work experience
• Experience conducting and documenting psychosocial assessments and interventions to meet physical, functional, social, emotional, educationalneeds of the patient/caregivers
• Experience in a Healthcare setting with community and national resource navigation and coordination, payment methodologies, mitigatingpsychosocial barriers to patient/family adherence

Preferred:

• 5-7 years experience in a pediatric setting
• Bilingual in English/Spanish
