This is a full-time hybrid Social Work Case Manager position based in Dayton, OH, with in-home visits taking place in Montgomery, Greene, and Clark counties. The Case Manager will coordinate and oversee the delivery of services for older adults, ensuring their needs are met effectively. Daily responsibilities include conducting assessments, developing care plans, connecting individuals to home and community-based resources, and providing advocacy.
The role also involves conducting follow-ups to evaluate service effectiveness and making required adjustments to care plans. Collaboration with external partners, caregivers, and other professionals is integral to the position.
- Must be a Licensed Social Worker (LSW) or Registered Nurse (RN)
- Strong communication, interpersonal, and advocacy skills
- Knowledge of community resources, social services, and aging-related challenges
- Organizational and time management skills for maintaining detailed records and managing a caseload
- Ability to work independently and in a team environment
- Proficiency in using technology, including case management software and standard office applications
- Previous experience working with older adults or in social services is preferred
