** Job Details:
** The Integrated Care Resource Specialist functions as a member of a multidisciplinary health care team with guidance from the Medical Social Workers, Nurse Care Managers, and Patient Classification and Denials Nurses. Assists in facilitating and linking patients and families with post-discharge placement needs. Can provide direct care specifically in need of resources that have been identified by the family, patient, or medical team and will work with them to identify and problem solve alternative options for obtaining community resources that may be a barrier to discharge.
This position will assist with receiving consults, screening needed services, and provide needed handoff information to Integrated Care staff to ensure timely care is provided to patients and families. Will organize information about community resources and share information with the department with updates on existing and new resources and services. Supports Child Passenger Safety Program by becoming a certified child passenger safety technician.
The Integrated Care Resource Specialist functions as a hospital liaison with external partner agencies and collaborates with medical teams, Center for Health Equity, Partners for Kids, Community Health Workers, Health information Management (HIM), and other hospital departments and as needed. This position will work with the leadership and staff on projects to support the Department. Position requires ability to manage various tasks and follow ups, can prioritize tasks in a diverse work environment and manage complex family dynamics as part of the Integrated Care Team.
Work is consistent with the mission, strategic plans, and policies of the organization, and will be conducted in collaboration with other departments to ensure appropriate and efficient usage of resources

* Education:
*** Bachelor’s degree required
* Bachelor’s degree related field (social work, psychology, or education) preferred
*
* Experience:
*** 1-2 years or related field experience required
* Experience in acute care hospital or outpatient clinical services preferred
* Experience in case management and/or denials prevention programs preferred
*
* Skills:
*** Exceptional oral and written communication skills required
* Team coordination skills required
* Computer literate, proficiency in Microsoft Word, Excel, PowerPoint required
* Knowledge of community resources, experience with children and families preferred
* Experience in the areas of quality improvement and patient safety preferred
* Epic Systems experience preferred
*
* Schedule:
*** 4 (10) hour shifts, 8: 00 AM - 6:30 PM
* Shifts will vary Monday - Friday, including Saturdays
