Gentiva Hospice is seeking a Patient Care Manager in Dayton to oversee patient care, lead clinical operations, and ensure regulatory compliance across the hospice program. Reporting to the Executive Director, you will manage visits, coordinate care, mentor staff, and lead IDG meetings, while upholding Medicare/Medicaid and ACHC standards.
Candidates must hold an RN license in the state, have 3+ years direct patient care experience including hospice/home health, and be able to lead a diverse care
