Physical Therapist (PT)
We are seeking a licensed Physical Therapist (PT) to join our home health team. The PT will provide individualized rehabilitation programs to help patients regain mobility, reduce pain, and improve overall physical function in a home care setting.
Key Responsibilities- Perform comprehensive physical assessments of patients
- Develop and implement individualized treatment plans
- Provide therapeutic exercises and mobility training
- Educate patients and families on recovery and prevention strategies
- Monitor patient progress and adjust care plans accordingly
- Maintain accurate and timely documentation
- Collaborate with interdisciplinary healthcare teams
- Degree in Physical Therapy from an accredited program
- Active Ohio PT license
- Strong clinical and assessment skills
- Home health or post-acute experience preferred
- New graduates welcome
- Setting:
Home Health - Patient Type:
Geriatric / Post-acute - Schedule:
Monday-Friday, 9:00 AM - 5:30 PM - Patient Load: 4-6 patients/day
- Travel: 25-30 mile radius (mileage reimbursed)
- Pay: $42-$58+ per hour
- Mileage reimbursement: $0.40/mile
- PTO, 401(k), pension, sick leave (eligibility applies)
