Aventura at Carriage Inn and Aventura at Shiloh Springs are seeking a Full-Time Physical Therapist (PT) to join our energetic and supportive rehab team. This split opportunity offers the chance to work across two nearby facilities while making a meaningful impact on the lives of residents every day.

Responsibilities- Evaluate and treat residents in a SNF/LTC setting
- Develop and implement individualized therapy treatment plans
- Help residents improve mobility, strength, balance, and overall function
- Collaborate with nursing, therapy, and interdisciplinary teams
- Complete documentation using Net Health and Point Click Care (PCC)
- Maintain compliance with facility and regulatory standards
- Licensed Physical Therapist (PT) in the state of Ohio
- SNF/LTC experience preferred, but not required
- New graduates are welcome to apply
- Strong communication and clinical skills
