Looking for a flexible PRN opportunity where you can focus on great patient care and be part of a supportive rehab team? Aventura at Carriage Inn is seeking a PRN Physical Therapist (PT) to join our in-house rehabilitation team!

This is a great opportunity for a PT who enjoys working with the skilled nursing and long-term care population and is looking for flexibility while making a meaningful impact on residents’ mobility, independence, and quality of life.

• Net Health for therapy documentation

• Point Click Care  (PCC) for facility documentation

• Evaluate residents and develop individualized physical therapy plans of care

• Provide skilled PT services to help residents improve strength, mobility, balance, and functional independence

• Collaborate with PTAs, nursing, physicians, and the interdisciplinary care team

• Complete timely and accurate documentation

• Participate in discharge planning and resident/family education

• Maintain compliance with facility, state, federal, and professional standards

• Graduate of an accredited Physical Therapy program

• Current Ohio Physical Therapist license in good standing

• SNF/LTC experience preferred

• Experience with Net Health and Point Click Care  (PCC) preferred

• Strong communication and teamwork skills

• Dependable, compassionate, and resident-focused
