Requirements
What You'll Do
Under the supervision of a Licensed Physical Therapist, you will:
- Deliver physical therapy treatments according to established plans of care
- Help patients improve mobility, strength, balance, function, and independence
- Communicate patient progress and concerns with the supervising Physical Therapist and interdisciplinary care team
- Collaborate with physicians, nursing staff, caregivers, and families to support patient outcomes
- Participate in care planning and interdisciplinary team meetings as appropriate
- Provide education and training to patients, families, and facility staff
- Complete timely electronic documentation, treatment notes, and daily billing activities
- Assist with discharge planning and continuity of care
- Support restorative and functional maintenance programs within the facility
- Maintain productivity and documentation standards while delivering high-quality patient-centered care
Qualifications
- Graduate of an accredited Physical Therapist Assistant program
- Associate degree as a Physical Therapist Assistant
- Current PTA license in the state of practice
- New graduates are encouraged to apply
- Strong communication, teamwork, and clinical problem-solving skills
- Passion for delivering exceptional patient-centered care in a Skilled Nursing Facility setting
