position summary:

under the direction of the fetal infant mortality review supervisor, the community health advocate i/ii is responsible for coordinating outreach for the every baby sleeps safe program, providing education, developing partnerships, and participating on program related subcommittees within montgomery county.

responsibilities include but not limited to:

  • serves a dual role by making home visits and serving as the coordinator for the every baby sleeps safe program which ensures infants have a safe sleep environment
  • establishes a network of partners for the every baby sleeps safe program
  • provides outreach and ensures that eligible families receive safe sleep education and a pack ’n’ play
  • tracks distribution and verifies eligibility requirements
  • promotes and ensures participation in sleep safe education seminars and ensures implementation of ohio department of children and youth’s safe sleep curriculum or other approved curriculum
  • participates on the ohio injury prevention partnership child injury action group safe sleep subcommittee and statewide safe sleep stakeholders calls
  • provides follow-up and assesses the improvement of safe sleep environments and impact of educational interventions
  • submits reports and ensures objectives are met

minimum qualifications:

  • bachelor’s degree in social science, behavioral scienceor related field
  • 2-3 years of community-based work experience in community health, community outreach, or social services
  • community health advocate i: initiation/completion of community health worker (chw) educational requirements within one year of employment
  • community health advocate ii: community health worker certification (chw)
  • proficient with microsoft word, excel, and power point
  • understands and acknowledges that public health - dayton & montgomery county uses evidence-based and evidence-informed public health practices based on scientific research including health promotion theory and practice, environmental health measures, and communicable disease control measures including vaccination, isolation, and quarantine practices, and medicine for treatment and prevention of disease, among others
  • understands and acknowledges the use of tobacco or nicotine products in any form including replacement therapy products and prescription medications used to treat smoking addiction for anyone hired after january 1, 2014, is prohibited
  • required to play an active role during a public health emergency, crisis, outbreak, incident, or event in which the health commissioner deems additional personnel resources are warranted. This may include changes in responsibilities and working locations/hours
  • completion of incident command system 100 and national incident management system 700 courses within 90 days of hire
  • valid state of ohio driver’s license and insurance on any personal vehicle that will be used for work
