Nurse Practitioner, Healthcare Nursing, Clinical Nurse Specialist, Advanced Practice Nurse/APRN
Our healthcare system is fragmented — leaving patients and caregivers to navigate critical transitions on their own. After discharge, they’re often left wondering:
- What medications do I actually take?
- What happens next?
At Mend Health, our mission is to bring patients clarity, connection, and coordination when it matters most.
We are helping piece the system back together by partnering with local hospitals, ACOs, and post-acute providers to create a more connected, supportive experience for every patient — right in their home.
We are seeking Nurse Practitioners who combine clinical excellence with independent judgment and a deep commitment to a patients experience. Clinicians who want to operate at the top of their license while delivering measurable impact in value-based care environments.
Our vision is to lead the way in restoring a healthier, more supported way of healing. We are not simply improving outcomes — we are reshaping what post-discharge care should feel like.
Role Description
Nearly half of Medicare patients require post-acute care after leaving the hospital — and a significant portion of total Medicare spending and avoidable readmissions is driven by what happens in the post-discharge period
That’s where Mend Health works.
