Position Summary

The Licensed Practical Nurse (LPN) provides direct nursing care to residents in a long-term care facility under the supervision of a Registered Nurse or Physician. The LPN is responsible for delivering safe, compassionate, and effective nursing care while supporting residents’ well-being and maintaining compliance with regulatory standards.

Key Responsibilities
  • Provide direct nursing care to residents according to care plans and physicians’ orders.
  • Administer medications and treatments accurately and in a timely manner.
  • Monitor and document residents’ health status, reporting any significant changes to the RN or physician.
  • Assist with resident assessments and the development of individualized care plans.
  • Supervise and direct CNAs and other nursing assistants in delivering daily care.
  • Ensure all documentation is complete, accurate, and in compliance with facility policies and state/federal regulations.
  • Respond to emergencies and follow proper protocols.
  • Support residents and families with health education and emotional support.
  • Maintain compliance with infection control, safety, and HIPAA standards.
  • Participate in interdisciplinary team meetings, audits, and in-service training.
Qualifications
  • Valid LPN license in the state of employment
  • Graduate of an accredited Licensed Practical Nursing program
  • CPR/BLS certification (required or able to obtain upon hire)
  • At least 1 year of nursing experience (long-term care preferred, but not required)
  • Ability to communicate effectively and compassionately with residents, families, and team members
  • Familiarity with electronic medical records (EMR) systems preferred
  • Strong critical thinking, organizational, and time-management skills
