Position SummarySiena Woods Care Center is a full-service nursing facility specializing in complex care. We are committed to providing high-quality clinical and rehabilitative services in a compassionate, supportive environment designed to meet the evolving needs of our patients and residents.
RNs are responsible for providing compassionate, high-quality care to residents while ensuring compliance with all regulatory requirements. This role involves direct clinical care, documentation, communication with interdisciplinary teams, and supervision of nursing staff.
Resident Care & Clinical Duties- Conducts
daily rounds
to assess residents, verify hand-off information, and ensure continuity of care. - Performs
comprehensive assessments
for new admissions and residents with changes in condition. - Delivers and oversees
direct nursing care
, including:- Administering medications.
- Recording vital signs and intake/output.
- Utilizing medical equipment and supplies as needed.
- Assists in the
development, revision, and monitoring
of resident care plans. - Ensures compliance with
regulatory standards
while supporting resident well-being—physically, socially, and emotionally.
- Notifies appropriate parties (physicians, families/legal reps, staff) of significant resident changes or incidents.
- Maintains
accurate and timely documentation
of care, assessments, and any unusual events. - Escalates relevant concerns to the
Director of Nursing
including regulatory issues, behavioral changes, and staffing challenges.
- Directly supervises
nursing personnel by:- Assigning duties and shifts.
- Evaluating staff performance.
- Training and onboarding new or temporary team members.
- Identifies staff development opportunities and provides
real-time coaching
to support clinical excellence.
- Assists in
survey preparation
and responds to audit-related activities. - Enforces compliance with
OSHA
, blood borne pathogen, and hazard communication protocols. - Participates in
emergency preparedness
including fire and disaster drills.
- Participates in
quality assurance programs
to maintain and improve care standards. - Helps
review and update facility policies and procedures
in accordance with regulations and best practices. - Stays current through
continuing education
and mandatory in-service training.
- Strong clinical judgment and assessment skills.
- Excellent interpersonal and communication abilities.
- Proven ability to supervise, lead, and support nursing staff.
- Physical ability to lift
up to 75 lbs
and perform other essential duties. - A commitment to maintaining
resident dignity, safety, and quality of life

- Directly supervises licensed and unlicensed nursing staff.
- Indirectly oversees other care team members as related to resident care.
- Participates in hiring, orientation, performance reviews, scheduling, and resolution of personnel issues.
Education & Experience
- Graduate of an
accredited nursing program
. - Minimum1 year RN experience
preferred. - Must meet all applicable
state and federal regulatory requirements
.
- Must hold and maintain an
active Registered Nurse (RN) license
in the state of employment.
