Hospice Nurse After-hours LPN

Salary: $31.25-$34.13 BOE

Schedule:

Full time |Mon/Tue/Thur 8P-8A

Location:

Dayton, Ohio, and surrounding area

• Licensed Practical Nurse (LPN) in state of practice
• A minimum of 1 year of acute care experience
• Hospice experience preferred
• Valid driver's License and Auto insurance coverage
• Reliable transportation:
  Willing to travel

• Provide skilled nursing care to patients in EOL, conducting visits in a facility and/or home patients.
• Provide on-call services after hours, and complete skilled visits.
• Assist RNCM in carrying out the plan of care.
• Assist the Nursing team in performing specialized procedures (as defined in the state nurse practice act) as directed by the RNCM under the physician's orders.
• Actively participate in the IDGH in collaborating to ensure the patient plan of care is kept current and identified issues are addressed.
• Assist in the reinforcement of patient and family education regarding end-of-life care.
• Assume responsibility for personal and vocational growth and development through active participation in nursing organizations and participating in in-service programs and other on-the-job learning.
• Keep the RNCM informed of changes noted during the process.
• Participate in self-development by practicing further education, workshops, and conferences, active in professional and related organizational meetings, and individual research and reading.
