Hospice Registered Nurse Case Manager

Salary: $80-89k/yr BOE

Schedule:

Full time | M-F | 8:00AM-5:00PM +(on-call rotation)

Location:

Dayton Ohio and surrounding areas

• Licensed Registered Nurse
• BLS Certified
• Valid Driver's License and Auto Insurance Coverage
• Reliable transportation
• A minimum of 1 year of Clinical experience
• Hospice Experience Preferred

• Coordinate hospice care by working with other team members as well as with the patient's family members to ensure needs are met.
• Evaluate the effectiveness of hospice services to the patient and family on an ongoing basis.
• Perform evaluations for hospice care along with informational visits as needed.
• Consult with the attending physician and hospice medical director concerning changes in patient condition requiring order changes and/or changes in the plan of care.
• Submit complete, accurate, and relevant clinical notes promptly.
• Participate in IDG and case conferences.
• Perform on-call duties per policy and need.
• Conduct an initial and ongoing comprehensive assessment of the patients.
• Develop and implement the initial plan of care and work with other members of the IDG in completing and maintaining the plan of care.
• Supervise and evaluate the care given by the hospice aide at a minimum of once every 14 days or as needed.
• Provide care plan to Hospice Aide and LPN as part of the IDG team.
• Perform death visits per company policy.
• Participate in patient's discharge planning process.
