Home Health Nurse (LPN or RN)

Job Summary

• Coordinate and provide patient care under the supervision of the Director of Nursing, following a physician‑established plan of treatment.
• Instruct patients and families in proper treatment procedures, medication management, and equipment use.
• Manage patient assessment, documentation, and discharge planning, ensuring effective communication with physicians, agency staff, and the multidisciplinary team.
• Apply critical care and infectious disease nursing skills to handle complex cases and support client care teams.
• Maintain professional nursing care within scope of practice, including personal care services, medication monitoring, client teaching, and emergency intervention.

Essential Functions / Areas Of Accountability

• Provide professional nursing care under the direction of a Registered Nurse.
• Perform skilled nursing visits as directed, following established care plans.
• Recognize and report changes in client condition to the Case Manager and physician.
• Communicate information in a timely, accurate manner through documentation and verbal updates.
• Conceptually apply safety, infection control, and equipment usage protocols.
• Promote a safe environment for clients and coworkers.
• Adhere to agency policies, state/federal regulations, and confidentiality standards.
• Participate in quality improvement activities and additional duties as required.

Typical Physical Demands

• Stand, walk, bend, stoop frequently.
• Squat/kneel, balance, sit occasionally.
• Push/handle 10‑20 lbs. occasionally; 20‑30 lbs. infrequently.
• Lifting 5‑10 lbs. occasionally; 10‑30 lbs. infrequently.

Requirements & Qualifications

• Active LPN or RN license in good standing, licensed to practice in Ohio.
• Minimum 1 year experience in a home health or equivalent in‑home care environment (preferred).
• Successful completion of HCN's written skills test and/or competency evaluation.
• Completion of all required state training/certifications for home health services.
• Active CPR and First Aid certifications (preferred to be current).
• Ability to perform required physical tasks safely and regularly.
• Strong background in nursing, patient care, safety, compliance, and confidentiality.
• Excellent attendance record with past employers.
• Good interpersonal skills and compassion for helping others.
• Ability to read, write, and follow detailed care plans and documentation procedures.
• Professional traits: reliable, honest, punctual, articulate, compassionate, detail‑oriented, accountable, respectful, self‑driven, problem‑solver, compliant, and confidential.
• Minimum 18 years of age.
• Valid driver’s license/ reliable transportation.
• Consent to pass a state and/or federal background check.

Desired But Not Always Required

• 3‑5 years of paid experience in home health or caregiving.
• Experience with Home Health Agency work.
• Specialized care: G‑tube, trach, ventilator/Trilogy, infants, persons with disabilities, or specific disease states.
• EMR/EHR proficiency, especially with Continu Link.
• Willingness to promote Home Care Network within community and networks, with eligibility for bonuses.
