Pay Rate: $29/hour plus shift differential

• Collaborate with the interdisciplinary team to deliver safe and quality nursing care in an at‑home environment.
• Administer medications and treatments, following personalized care plans.
• Maintain accurate and timely documentation of all nursing activities.
• Provide exceptional customer service to residents, families, and medical partners.

• Valid, unencumbered state LPN license.
• Minimum of 1 year as an LPN.
• Experience in assisted living or memory care is a bonus; a collaborative spirit is essential.
• Commitment to integrity, reliability, and a strong work ethic.
• Must successfully pass a pre‑employment drug screen, physical exam, and background check.
