Registered Nurse Care Manager- Summary
As the RN Care Manager, you will assess participants’ care needs, provide nursing and healthcare interventions, and evaluate outcomes on an ongoing basis. You will collaborate with the interdisciplinary team (IDT) to develop and implement care plans, delegate tasks to staff, and provide direct care across clinic, center, and home settings.
Essential Duties and Responsibilities- Provide high‑quality clinical care as a member of the PACE IDT.
- Deliver nursing care in the center, clinic, contracted facilities, and participants’ homes per individual care plans, documenting services in the medical record.
- Participate in the 24/7 “on‑call” triage process for participant needs.
- Assess, plan, and coordinate participants’ home‑care services and provide input to the IDT on home‑care plan interventions.
- Monitor acute and chronic care needs in all settings, coordinating care to promote continued community care or optimal institutional care as required.
- Ensure timely follow‑up by providers, and assist with obtaining specialist reports, facility documentation, and labs.
- Reconcile facility MARs for assigned participants monthly, ensuring accuracy and medication adherence, and notify providers of discrepancies.
- Notify participants of normal test results.
- Complete timely and accurate nursing assessments per policies and regulatory requirements.
- Implement nursing‑related care plan interventions.
- Teach participants, caregivers, and families about self‑care, medications, healthy lifestyles, infection control, and safety to promote optimal health and safety.
- Review and revise goals and approaches to participants’ care with participants, families, caregivers, and the IDT.
- Collaborate with the IDT to develop and implement comprehensive care plans.
- Develop and maintain positive relationships and communication with co‑workers, participants, families/sig‑plus others in the community.
- Participate in all IDT meetings.
- Assist IDT members in understanding significant nursing and functional needs related to participants’ health problems.
- Support OT by back‑up to Home Care Coordinator duties when needed, and perform other IDT member duties as required.
- Participate in utilization review meetings and quality improvement projects.
- Evaluate the competence of CNAs and Home Care Aides, delegating tasks as indicated.
- Participate in family meetings, staff meetings, in‑service training, and orientation programs as required.
- Follow all PACE Program Policies, OSHA safety guidelines, and infection control protocols.
- Protect privacy and maintain confidentiality of all company procedures and information about employees, participants, and families.
- Practice standard precautions.
- Perform other duties as required or requested.
- Bachelor of Science in Nursing degree preferred.
- State RN license required.
- New Jersey licensure by the NJ State Board of Nursing (if applicable).
- BLS required (certified within 90 days of employment).
- 1 year of experience working with a frail or elderly population preferred; training provided if not present.
- Experience in home care, long‑term care, or managed care preferred.
- 1 year of RN care experience required.
- Reliable transportation, valid driver's license, and minimum state‑required liability auto insurance.
- Medical clearance for communicable diseases and up‑to‑date immunizations before direct participant contact.
- Comprehensive criminal background check, including exclusion lists, criminal history, education verification, license verification, reference check, and drug screen.
- Required immunizations.
