Position: PDN / Private Duty Nurse (LPN or RN) - Home Health, 8hrs/wk or Part-time

JOB TITLE:

Home Health Visit Nurse (RN or LPN)

Active Ohio LPN or RN in good standing is required.

  • At least 1 year (or 2–5 years preferred) of paid experience at a home health agency or equivalent.
  • Experience desired in case management, wound care, PICC/PORT/IV care, lab draws, OASIS/Non‑OASIS assessments; nurse supervision a plus.
  • 1+ year in home care with vent care required.
SCHEDULE TYPE & SERVICE AREA:
Dayton & Metro Dayton, Ohio
  • Typical availability Monday‑Friday 8 am–5/6 pm.
  • Part‑time PDNs: travel radius within Montgomery County and surrounding 30–40 minute radius (up to 60 min on occasion).
  • Full‑time PDNs: travel radius of up to a 2‑hour radius into surrounding counties; additional cases can be added if desired.
  • Qualified applicants may reside anywhere provided travel requirements are met.
EMPLOYMENT TYPE
  • Long‑term employment for full‑time or part‑time positions.
  • No on‑call, overnight, or required weekend shifts.
  • Overtime, respite, and PRN services offered when available but not required.
COMPENSATION
  • LPN: $28–$30 per hour.
  • RN: $35–$40 per hour.

Compensation is based on qualifications, certifications, experience, skill specializations, positive employment history, and client fit.

QUALIFICATIONS
  • Graduate of an accredited nursing school.
  • Active LPN or RN license in Ohio.
  • Minimum 1 year in home health industry and nursing experience; additional home health nursing experience desired.
  • CPR and First Aid certificates preferred; agency provides recertification.
  • Doctor’s prescription for vent care; minimum 1 year direct patient experience with vent care required.
  • Strong interpersonal communication skills (written and verbal).
  • Reliable, honest, professional, compassionate, detail‑oriented, accountable, friendly, respectful.
  • Valid driver’s license or state  reliable transportation required.
  • Excellent attendance record.
  • Ability to pass state and federal background check.
POSITION SUMMARY

A Successful PDN performs skilled nursing functions in accordance with federal, state, and local laws and agency policies. All care is delegated by the RN Case Manager.

REPORTS TO

RN Case Manager;
Clinical Administrator / Director of Nursing.

ESSENTIAL FUNCTIONS / AREAS OF ACCOUNTABILITY
  • Provides professional nursing care under RN direction, communicating status changes.
  • Delivers skilled nursing visits per RN Case Manager and follows care plans.
  • Recognizes and reports changes in client condition to the Case Manager and physician.
  • Communicates information timely via verbal and documentation.
  • Performs personal care, assessments, medication monitoring and administration, client education, and emergency interventions within scope.
  • LPNs with “M-IV” designation may perform venipuncture for lab samples.
  • Promotes personal safety and safe environments for clients and coworkers.
  • Adheres to safety/infection control policies.
  • Uses equipment safely and makes safety improvement recommendations.
  • Complies with agency policies, professional standards, and laws.
  • Accepts professional nursing responsibility, attends required meetings, maintains confidentiality, and participates in quality improvement initiatives.
ESSENTIAL PHYSICAL DEMANDS
  • Frequent standing, walking, bending, stooping; occasional sitting.
  • Handling weights: push/pull 10–20 lb occasionally, 20–30 lb infrequently; carry/lift 5–10 lb occasionally, 10–30 lb infrequently.
