Position: PERINATAL OUTREACH NURSE/MVH/FT

Perinatal Outreach Nurse (PON) Overview

The Perinatal Outreach Nurse is responsible for providing comprehensive care from pregnancy through the child’s second year, focusing on home visiting, education, and service coordination to promote family well‑being and child development.

Key Responsibilities
  • Conduct regular home visits to assess vitals and family needs, support parenting practices, provide education, and make referrals, while monitoring child development.
  • Perform detailed assessments:
    • Pregnant women: weight, blood pressure, edema, fetal movement, breasts, heart and lung auscultation, signs of preeclampsia.
    • Postpartum women: BUBBLE assessment (breasts, uterus, bladder, bowels, lochia, incision), vitals, infection signs, emotional well‑being, bonding, pain.
    • Newborns: weight, length, skin, eyes, heart and lung auscultation, abdomen, genitals, nervous system, reflexes, feeding method.
  • Deliver structured, evidence‑based curriculum during visits to promote school readiness, parent‑child interaction, healthy development, and overall family wellbeing.
  • Develop and monitor individualized service plans addressing education, employment, health, housing, and child development goals.
  • Connect families with community resources including prenatal care, lactation, postpartum care, pediatrics, dental, vision, hearing, developmental, housing, employment, education, legal, primary care, and mental health services.
  • Maintain accurate, timely records of all interactions, assessments, referrals, and follow‑ups in accordance with program guidelines and confidentiality standards.
  • Advocate on behalf of families to ensure access to services and promote self‑sufficiency and empowerment.
  • Ensure service delivery aligns with agency policies, funding requirements, and applicable regulations.
  • Foster strong, supportive relationships with families to encourage ongoing participation and engagement in services and child development activities.
  • Collaborate closely with interdisciplinary teams (educators, health professionals, supervisors) and external stakeholders to coordinate care.
  • Support families through transitions, including entry to preschool, childcare, employment, or other programs.
  • Participate in team meetings, supervision, and professional development to enhance service quality and stay informed of best practices.
  • Engage families in group activities or workshops (parenting classes, health education, family engagement events) when applicable.
  • Collaborate with leadership to identify learning needs and formulate professional development plans.
  • Assist with mentoring home visitors, service coordinators, and other Perinatal Outreach Nurses.
  • Licensure & Certification
    • Licensed Registered Nurse in Ohio.
    • Must undergo a criminal background check with the Ohio Bureau of Criminal Identification and Investigation and the Ohio Central Registry on Child Abuse and Neglect.
    Education & Experience
    • Minimum Bachelor’s degree in Nursing (Bachelor’s preferred; ADN may be considered if an MOU is signed to obtain a BSN within 5 years of hire).
    • 1–3 years of job‑related experience; at least one year working with pregnant women, infants, and children is preferred.
    • Prior experience in home visiting, OB/Gyn, Pediatrics, Adolescents, or Mental Health settings is preferred.
    • Complete orientation based on Nurse Family Partnership and Help Me Grow guidelines.
    Knowledge, Skills & Requirements
    • Effective verbal, written, and interpersonal communication skills.
    • Organized, self‑directed, and able to manage multiple client needs.
    • Valid driver’s license and insured safe automobile.
    • Non‑smoker.
    Reporting & Supervision

    The PON reports to a Nurse Supervisor for weekly supervision and professional development and is accountable for compliance with all applicable regulatory standards, policies, procedures, and protocols.
