The Perinatal Outreach Nurse (PON) is responsible for providing comprehensive care for pregnant and postpartum clients and children up to two years of life. Primary duties include:
- Conduct regular home visits to assess vitals, family needs, and support parenting practices, and deliver evidence‑based curriculum on child development and family well‑being.
- Perform detailed assessments for pregnant women, postpartum women, and newborns, including weight, blood pressure, fetal movement, and newborn physical and emotional status.
- Develop, implement, and monitor individualized service plans and coordinate referrals to community resources such as prenatal care, lactation services, pediatrics, dental, housing, employment, and mental health.
- Maintain accurate, timely documentation of all interactions, assessments, referrals, and follow‑ups in accordance with program guidelines and confidentiality standards.
- Advocate for families to ensure access to services and promote self‑sufficiency and empowerment.
- Ensure compliance with agency policies, funding requirements, and applicable regulations, including mandated reporting laws.
- Build strong, supportive relationships with families, encouraging ongoing participation and engagement in services and child development activities.
- Collaborate with multidisciplinary teams (educators, health professionals, supervisors) to support coordinated service delivery.
- Assist with mentoring home visitors, service coordinators, and fellow Perinatal Outreach Nurses.
- Minimum education:
Bachelor's degree in Nursing; consideration for ADN holders who will obtain a BSN within five years of hire through a signed Memorandum of Understanding. - Required background:
Criminal background check with the Ohio Bureau of Criminal Identification and Investigation and a child‑abuse/neglect registry check with the Ohio Central Registry. - Licensure:
Licensed Registered Nurse (RN) with active licensure from the Ohio Board of Nursing. - Experience:
1–3 years of job‑related experience, preferably in home visiting, OB/Gyn, Pediatrics, Adolescents, or Mental Health settings. - Preferred:
At least one year working directly with pregnant women, infants, and children. - Other requirements:
Complete orientation per the Nurse Family Partnership and Help Me Grow guidelines. - Knowledge &
Skills:
Effective verbal, written, and interpersonal communication; organized and self‑directed; valid driver’s license and insured safe automobile; non‑smoker. - Preferred educational qualifications:
Must undergo the required criminal background checks.
