Care 360 Hospice is hiring a Hospice Registered Nurse Case Manager to join our field team in the Dayton area. The position is full-time, M-F 8:00 AM-5:00 PM with on-call rotation. Travel to patient residences in Kettering, Ohio, and surrounding areas is required.
You will assess patients, develop care plans, coordinate with the IDG, educate families, and document promptly. We seek an RN with at least 1 year of clinical experience and BLS certification; hospice experience is preferred.
