Care 360 Hospice is seeking a registered nurse to join our Hospice Registered Nurse Case Manager team. You will travel to patients' homes in Troy, Ohio and surrounding areas to assess needs, review medical histories, and develop individualized care plans with the IDG.
You will coordinate hospice services with the patient’s family and healthcare team, educate families, document care, and participate in on-call rotation.
